In the fifth season he ends up formalizing his relationship with Lexie and in the sixth he discovers that he has an 18-year-old daughter that he never wanted to take care of, which makes his relationship with "Little Gray" difficult. Although he later begins a brief relationship with Teddy Altman, he is still in love with his best friend's wife's half sister. In the seventh season he returns with lexie but it ends with him again, upon learning that he is the father of Callie's baby. Meredith's half-sister from the father enters as an intern in the fourth season under Cristina's orders. Sentimentally, he first starts an affair with Alex, to fall in love after his friend George. When she discovers that he only sees her as a friend, she begins a relationship with Mark Sloan. Everything is going well until the arrival of his pregnant daughter, which causes a breakup, after which she will again have an affair with Alex. When he returns with Mark, he discovers that he has made Callie pregnant and puts an end to the relationship, starting another with Avery.

Daughter of a renowned surgeon, Meredith is the protagonist of the series. He maintains a relationship with Derek and is the narrator of each episode. His mother, an eminent and renowned doctor, author of a book on anatomy, dies of Alzheimer's. His father, with whom he has no relationship, reappears in his life and his half-sister Lexie enters as a hospital intern. She is married to Derek after many comings and goings and finally plans to be a mother, so in the end they adopted an African girl named Zola. He is a reputed neurosurgeon who moved from New York to Seattle after his wife, Addison, cheated on him with his best friend, Mark Sloan. He is in love with Meredith from the first moment he saw her. At the end of the third season he rejects the position of Chief of Surgery at Seattle Grace to have a better relationship with her, after seeing what had happened in the chief's marriage. It is known as "McDreamy" (Doctor Massif) among residents. In the fifth season he marries Meredith and in the middle of the sixth he replaces Richard as chief of surgery. He discovers that he is not as he believed and at the end of the season he is shot in the chest that makes him reflect on life, so he renounces the head of surgery and returns to neurosurgery. In the seventh season he tries to have a son with Meredith but she can't, so they decide to adopt a baby.
